Confidentiality

All counseling session content is confidential unless the client provides a written request to have all or portions of session content released to a specific named person(s). Limitations of client privilege include:

  1. If a client threatens death or serious bodily harm to him or herself or another person.
  2. If the therapist has a reasonable cause to believe the client or another named party is the perpetrator, observer, or victim of physical, emotional, or sexual abuse or neglect of a child under the age of 18 years.
  3. Suspicions stated above in the case of an elderly person(s).
  4. If a court of law issues a subpoena for information stated on the subpoena.
  5. If the client is being treated by order of a court of law, or if information is obtained for the purpose of rendering an expert’s report to an attorney.

Cancellations

Counseling sessions will begin on and at the agreed upon time. Sessions will be canceled if the client is delayed by 18-minutes. If you think you will be late, please notify your therapist via telephone or text. When delayed, the session will end at the normal scheduled time. If the delay is due to the therapist, he or she will provide services for the full time. If you must cancel an appointment, please give at least 24 hours advance notice via email or text. Forgotten appointments or last-minute cancellations will incur the usual service fee.

Good Faith

As of January 1, 2022, if you are uninsured or choose to pay for health care out of pocket, health care providers and facilities must provide a good faith estimate upon request.

Insurance

Life’s Passages is fee for service and does not accept insurance.  You can learn what mental health services your insurance policy covers by contacting your insurance carrier. Any arrangements you have with your insurance company is a contract specifically between you and them. Life’s Passages can provide you with a monthly statement to submit to your insurance company upon request.

Payment Policy

Complete payment for services is required on the day services are provided. Payments can be made with a personal check (Life’s Passages), credit card, Venmo, or CashApp. Returned checks will incur a $50 penalty. A statement for services is available upon request.
